Informed Consent Form WFU Leadership, Character, & Ethics Poll

You are invited to participate in a research poll study. Please read this consent agreement carefully before agreeing to take part.

Purpose of the research poll:

The two major purposes of the poll are to 1) Assess the state of leadership, character, and ethical attributes of America’s business and organizational leaders and 2) Inform a national conversation on actions needed to bolster the capabilities and character of America’s business and organizational leaders.

What you will do in this study:

Participation in this study involves completing 8 questions.

Time required:

Your participation will take approximately 15 minutes.

Risks:

The risks from participating are not more than would be encountered in everyday life.

Benefits:

There is no direct benefit from your participation in this study; however, we hope the study will be an investment in developing organizational leaders dedicated to achieving results with integrity.

Confidentiality and Privacy:

The study team will take the following precautions to protect your privacy and the confidentiality of your information.  The data are SSL encrypted, the information is stored in a password-protected database, and IP addresses are not collected. These measures provide a very high level of security used by financial institutions, and it is very unlikely that your data could be accessed by anyone. All responses are stored on Qualtrics online survey software and password-protected computers authorized to be used only by individuals on this proposal. However, email and the Internet are not 100% secure. When completing the survey, please choose a location with adequate privacy and internet security. We also suggest you clear the computer’s cache and browser history to protect your privacy after completing the survey. The results from this Wake Forest University research poll will be reported in the aggregate; thus, no individual responses can be identified.

We may publish/present the results of this research. We may share your research data with other investigators without asking for your consent again. Any information we publish, present, or share will not contain information that could directly identify you.

Participation and withdrawal:

Your participation in this research is voluntary. If you do not wish to participate, please close your browser window.  You may discontinue your participation without penalty by closing your browser window. Any responses entered to that point will be deleted. You may also choose not to answer any question(s) you do not wish to answer for any reason.
